Digital files can be uploaded to the My Account portal. If you are a current customer not registered for this feature, or if you are a new doctor, call us at 800-854-7256 to set up your account.

An Rx is required for every case you send us. Download and use this Universal Rx for most general cases
Patient case photos can make the difference between creating a great restoration and an extraordinary one. Our specialized team of dental technicians will use your case photos to help them treatment plan your case.
Shipping and Packing a Case Box
- Place foam on bottom of box.
- Separate models, and secure impressions and bites.
- Arrange contents in a plastic bag for proper infection control.
- Place foam on top of contents.
- Add prescription.
- Tape box securely shut.
- Attach a preprinted shipping label to top. (Please note that you must use a pre-printed Glidewell shipping label to avoid being billed by FedEx at a much higher cost.)
Note: If you are sending a scheduled rush case, write “RUSH” on the outside of the box/package as well as on the prescription. To schedule a rush case, please contact Customer Support at 855-289-9657.
Ship your case to Symmetric Smiles using FedEx overnight service and pay just $9 each way ($18 round-trip per box; contiguous U.S. only).
Use our prepaid USPS Priority Mail shipping labels at no charge, and your case will arrive to the lab in 2-4 business days and back at your office in 2-4 business days.
- Time of case pickup and delivery may affect turnaround time.
- To save money on shipping multiple cases, check out our bundling program and combine multiple cases into one box.
Rush service must be scheduled by calling 855-289-9657 before the case is shipped to guarantee that rush service for your case will be available.
